If your family is experiencing difficulties and you would like to receive help from Life Changing Counselors please first call PerformCare at (877) 652-7624. Describe your family's current issues and be sure to provide accurate information about mental health or behavioral problems. If it is determined that your family would benefit from intensive in-home services, you may request Life Changing Counselors as your provider.
